Senior Day Health Center

The Senior Day Health Center is a daytime program which encourages the vitality, independence, health and well-being of less independent older adults while supporting and providing respite to their families and other care givers. The Center is designed to improve participants' level of functioning through activity programs that are stimulating and fun. SDHC also offers daytime respite and support for care givers who may need relief from day-to-day care of their elders.

Individualized care plans are designed to meet the specific needs of each participant in consultation with the individual's physician. A hot noon meal is part of the daily schedule and special diets can be provided for those who need them. Free transportation is provided within the service area of Los Altos, Mountain View, and Palo Alto.

Counseling for families and a monthly family support group are included in the services offered by the staff of social workers, physical, occupational and speech therapists, nurses, and activity program specialists. Referrals to other community agencies are provided as needed.

The Center is open to enrolled participants five days a week from 8:00 a.m. to 5:00 p.m. Participants may attend from 2 to five days a week.

SDHC is located at 701 East Meadow Drive (at Middlefield), Palo Alto, in the facilities of the Palo Alto Baptist Church.

The Center has a sliding fee scale based on avaliable income. The Program is reimbursed a set fee by the Medi-Cal program for eligible participants. Fees are partially subsidized by the City of Palo Alto for its residents who meet income requirements.

The County of Santa Clara allocates funds for the Center and the United Way of Santa Clara County provides funding support through the Senior Coordinating Council of the Palo Alto area; supporting funds are raised from the special events of the Association for Senior Day Health. Special gifts from foundations, corporations, service clubs and individulas are solicited.

Memorial gifts or donations honoring birthdays and anniversaries are most welcome.

The Senior Day Health Center is a program of the Senior Coordinating Council of the Palo Alto Area, Inc. It is licensed by the Sate of California, Department of Health Services.

The staff follows up promptly on referrals and inquiries, which usually come from potential participants, their spouses, family members, physicians, social agencies and churches.
